We are committed to helping you build those vital links, which is why we can work with you to develop a personalised Community Connections Plan. This service is available to everyone, even if you are not an NDIS participant.

The goal of community connections is simple: to help you find and access the valuable supports available in your local area. These fall into two main categories:

  • Mainstream Supports: These are the services provided by other government departments, such as the healthcare system, mental health services, and education providers. We can help you navigate these systems to get the support you need.
  • Community Supports: These are the diverse resources offered by community organisations. This can include anything from peer-to-peer support networks and local hobby groups to services run by local councils or faith-based organisations.

Creating your plan is a flexible and collaborative process. For some, learning about and connecting with these supports may only take one or two meetings. For others, it might be a regular conversation as your goals and needs evolve. At Disability Choice Matter, we’re your dedicated partner in this journey, and there is no limit to the number of times you can contact us for guidance and support.

If you think you are eligible and want to apply to become an NDIS participant, there are different ways to complete your NDIS application. You can choose the way that best suits you. We provide comprehensive support to ensure you get the right NDIS funding.
