Navigating Your NDIS Application: A Clear Guide

Applying for the NDIS involves several key steps and requirements. The process can seem confusing, but the team at Disability Choice Matter is here to provide expert guidance and hands-on support from start to finish.

Understanding the NDIS Pathways

The first step is to understand the correct pathway based on age.

  • For children younger than 9: The NDIS is accessed through the Early Childhood Approach. We can help you prepare to meet with your local Early Childhood Partner, ensuring you have the right information to support your child.
  • For individuals aged 9 – 65: You will need to meet specific NDIS access requirements related to your disability and its impact on your life. This is our core area of expertise, and we can help you build a comprehensive application to demonstrate your eligibility.

How We Support You

No matter the pathway, our role is to be your advocate and guide. We simplify the process by assisting you to:

  • Understand the Criteria: We’ll walk you through the NDIS access requirements and help determine your eligibility.
  • Complete the Application: We provide hands-on help to fill out the NDIS Access Request Form (ARF) accurately.
  • Organise Your Evidence: We guide you in gathering the crucial medical reports and supporting documents needed for a strong application.

Submit with Confidence: We’ll help you compile and submit your completed application through the correct channels.

Completing the Access Request Form (ARF)

The ARF is the official application document. We’ll ensure it’s completed correctly, paying close attention to details like:

  • Health Professional Input: We can liaise with your treating health professionals to ensure they complete their required section of the form.
  • Digital Signatures: If you’re filling out the form digitally, we’ll make sure it’s signed correctly as the final step.

Telephone Requests: If you don’t have access to an NDIS partner or are unable to complete a form, the NDIA can be phoned on 1800 800 110 to make a Telephone Access Request. We can help prepare you for this call.

Gathering Your Supporting Information

A strong application is built on strong evidence. We will help you take the time you need to gather clear supporting information that confirms your:

  • Age (e.g., birth certificate, passport)
  • Residency Status (e.g., proof of Australian residency)
  • Disability or need for Early Intervention (e.g., reports from doctors or specialists)

We know exactly what the NDIS looks for and will help you organise these documents effectively.

Returning Your Application

Once the Access Request Form is completed, signed, and all supporting information is gathered, it’s ready for submission. You can return it by:

  • Email: to enquiries@ndis.gov.au (This is the fastest method).
  • Post: to GPO Box 700, Canberra, ACT 2601.

We will help you package everything correctly and can assist in sending the application on your behalf, ensuring you have a trusted email address for all correspondence from the NDIA.

What if More Information is Needed?

Sometimes the NDIA will request more details to make a decision. If this happens, they will ask for a Supporting Evidence Form to be completed by your treating health professional. This is a normal part of the process, and we will be right there with you to help manage this request quickly and efficiently.
